Terms and Conditions for GP Exercise Referral Scheme

1. Introduction
These Terms and Conditions govern your participation in the GP Exercise Referral Scheme (the "Scheme). By participating in the Scheme, you agree to comply with these Terms and Conditions. The Scheme is designed to provide a 12-week exercise program for patients referred by their General Practitioner (GP) to help improve health and wellbeing.

The correct rate for the 12-week programme is £75. Fees are payable in advance once you have been accepted onto the Scheme.

2. Eligibility
2.1. To be eligible for the Scheme, you must be a registered patient with a participating GP practice.
2.2. You must be referred by your GP or another healthcare professional involved in your care.
2.3. The Scheme is intended for patients who would benefit from increased physical activity due to a medical condition or risk factors identified by their GP.
2.4. Current or previous members of Atrium Fitness will not be eligible. The scheme is intended to be an introduction to a carefully managed exercise program, not a subsidised membership.
2.5. You will be eligible for one 12-week cycle on the Scheme as it is intended to be a way of introducing you to exercise. Once you have completed the 12-week programme you will have a variety of memberships you can sign up for at Atrium Fitness to carry on exercising in the future if you choose.

3. Referral Process
3.1. Your GP will assess your suitability for the Scheme based on your medical history and current health status.
3.2. If deemed appropriate, your GP will complete a referral form and provide relevant medical information to the Scheme’s coordinators. Referral forms will be honoured up to 3 weeks after the date your GP issued it.

3.4. You will be contacted by the Scheme’s coordinators to arrange an initial assessment and to discuss your exercise plan.

4. Initial Assessment
4.1. An initial assessment will be conducted by a qualified exercise professional.

4.2. The assessment will include a review of your medical history, fitness level, and any specific requirements or limitations.
4.3. Based on the assessment, a personalised exercise program will be developed to meet your individual needs and goals.

5. Exercise Program
5.1. The exercise program will be designed to improve your overall health and well-being.
5.2. The program may include various types of physical activities such as aerobic exercises, strength training, flexibility exercises, and more.
5.3. You will be provided with guidance and support throughout the duration of the program by qualified exercise professionals.

6. Attendance and Commitment
6.1. You are expected to attend scheduled sessions and actively participate in the exercise program that has been designed for you.
6.2. If you are unable to attend a session, you must notify the Scheme’s coordinators in advance.
6.3. Regular attendance and commitment are essential for achieving the desired health benefits.

7. Health and Safety
7.1. Your safety is our priority. You must follow all instructions provided by the exercise professionals.
7.2. Inform the exercise professionals of any changes in your health or any discomfort experienced during exercise.
7.3. You should wear appropriate clothing and footwear for physical activity.

8. Confidentiality and Data Protection
8.1. All personal and medical information provided will be kept confidential and used solely for the purpose of the Scheme.
8.2. Information may be shared with relevant healthcare professionals involved in your care.
8.3. Your data will be stored securely in accordance with data protection regulations.

9. Termination of Participation
9.1. Your participation in the Scheme may be terminated if you fail to comply with these Terms and Conditions.

9.2. Atrium Fitness reserves the right to refuse participation in the Scheme. 

9.2. You may withdraw from the Scheme at any time by notifying the Scheme’s coordinators.
9.3. Any fees paid for the Scheme will be non-refundable.

10. Liability
10.1. The Scheme and its coordinators will not be liable for any injuries or health issues arising from your participation in the exercise program, except where caused by negligence.
10.2. You participate in the Scheme at your own risk.

11. Amendments
11.1. The Scheme reserves the right to amend these Terms and Conditions at any time. Any changes will be communicated to you in writing.
